Your facility could be at RISK for a resident elopement IF ...

1. A resident has a diagnosis of dementia, displays known cognitive impairment, and is able to be independent with his/her mobility.
|
2. A resident is prone to wander.
|
3. A resident displays signs of restlessness and/or agitation, and is independent with mobility, or requires minimum assistance from staff.
|
4. A resident is actively looking for an exit from his/her particular unit, and/or facility.
|
5. A resident often asks questions about the rules in coming and going from the facility, and is independent with mobility or needs only minimum assist from staff.
|
6. A resident displays a past history of unsafe wandering and/or leaving a particular unit or facility without the required supervision.
|
7. A resident shows signs of poor adjustment to his/her particular unit or facility, and is independent with mobility or requires minimum assist from staff.
|
8. A resident is vocalizing the desire to leave his/her unit of the facility, and is independent with mobility or requires minimum assist from staff.

If you can identify a resident with any of the above characteristics, then you do have a resident at risk for wandering and elopement. |
